Centerville Fitness Liability Waiver and Release
Acknowledgment of Risk:
I, {name} , acknowledge that my participation in physical activities, including but not limited to exercise, training, and fitness classes at Centerville Fitness, involves inherent risks, including but not limited to the risk of injury or death. I understand that these risks may arise from my own actions or the actions of others, the conditions of the facilities, or any equipment used.
Release of Liability:
In consideration of my participation in the activities offered by Centerville Fitness, I hereby waive, release, and discharge Centerville Fitness, its owners, employees, instructors, agents, and affiliates from any and all claims, liabilities, damages, costs, or expenses arising out of or in connection with my participation in these activities, including those caused by negligence.
Medical Disclosure:
I certify that I am in good health and do not have any medical conditions that would prevent me from safely participating in physical activities. I understand that it is my responsibility to consult with a physician before beginning any new exercise program.
Indemnification:
I agree to indemnify and hold harmless Centerville Fitness from any claims, actions, or demands that may arise from my participation in activities at the facility.
Photography Release:
I grant Centerville Fitness permission to use photographs or videos taken during my participation in gym activities for promotional purposes, including social media and marketing materials.
Dress Code Acknowledgement:
I acknowledge that shirts and shoes must be worn at all times. No sandals or flip-flops are permitted in the gym.
Acknowledgment of Understanding:
I have read this waiver and release of liability carefully and understand its contents. I am aware that by signing this document, I am giving up certain rights, including the right to sue. I voluntarily agree to the terms and conditions stated herein.
